What impact is made if one changes a web page title or file name, if already in search engines? I’d like to clean up/tweak some page titles, and perhaps some file names, but am concerned it may negatively impact the site as it’s already indexed by many of the search engines.

That is a risk you may have to take.
It really depends what you have just now - if you have generic names/titles like Home, Contact etc. then I would say it would be advantageous to change them.
