An image in a set of 90 in Showcase gets replaced by another five images further on in the sequence so one image disappears - though its caption remains - while the other appears twice, once with the wrong caption. This has happened twice. The duplicated image is captioned John Watts and then John Walsh in the current case (the similarity of the names is not relevant I think as in the previous case there was no such similarity).
