Your form is very simple, if your server permits the ioncube loaders then follow the easiForm tutorial you should have your form up and running very quickly.

If you plan to use the PHP Feedback Form action
(http://www.freewayactions.com/product.php?id=019) then you should make sure
that all of the items you want to encapsulate in the form are all either on the
same layer or aren’t layered items. Having them on separate layers causes the
html items to get split up in the code making it that much harder for the
action to locate the tags and wrap the form item around them.
This will also help with your tab ordering between the form elements although
you can also specify the ordering manually; How to Control Tab Order in HTML – WebCheatSheet

If you use easiForm you can by pass ‘setting the form up’ as the action does all the form set-up for you as it explains in the tutorial, all you need to do is follow the easiForm tutorial to use it… nothing else… it is that simple.
