Quite some time ago, this thread, but I give it a try. (didn’t find a solution anywhere in TheList)
Problem: Tim Plumb’s form does not work.
error: server failure
I did use TP action several times, it’s my favourite. But on this new website it does not work. I get messages back :server failure. The helpdesk sais:
“Webserver is a Windows2003 server and we have
PHP5.2.4 installed.”
I use to create forms like described below.
Anyone a clue?

not an exact answer to your question, but if you send me off list the url from the formpage, I can make a (forms to go) form.php.
Maybe this will work.

This is a fundamental problem with PHP on WIndows (if not Windows
itself). Have a look here for some advice on the subject:

So, bottom line, TP’s PHP script is not working here?
How about other PHP based solutions, like the famous ExpressionEngine? I did run a test script on that location, EE sais that it’s OK to run EE there…?

I had contact with b8. The form he was so kind to send me had to be altered in order to function, but doing so without the original app FormsToGo was not easy.
So, Thanks, Paul, for the tip on the SP site. I’ll have a serious look at it.
One thing I do not understand, it’s in the Fun part of the site. huh? It’s no fun at all…;~(
David B, another option you have is easiForm, you can set it all up before buying it and you get built in protection from certain types of spam that might be attempted to be sent from your form.
