What is the graphic element for
All you need to create is the form input items
Start by drawing a table on the page 2 cols by however many rows you are going to have inputs for a 1 for the reset and submit buttons
Put your title (eg Name) in 1st column and then in second column click in the cell and Insert > Text Field (or area if it is to contain more text than the single line)
Repeat for next row 1st Col Address 2nd Col Insert>Text Area
and so on
In the last row insert a Submit button and a Reset Button (Insert>Button then change its parameters in the inspector)
Apply the PHP Feedback Form action to the table that holds all these fields and set the Action Parameters in the action inspector.
Remember to name your input fields following a logical selection ie name, address, email, question (the action pays attention to some of these like name and email)
