PHP Feedback Form Help Please!!

Good Evening,

I have been tinkering around with the PHP Feedback Form a good portion of the day, looked up help via here and other places and unable to find a step by step tutorial unfortunately.

First off, does anyone have a good link to a step by step instructions .

If not, here’s my problem. I am trying to do a contact form using Tim’s PHP Feedback Form. I am probably doing something wrong along the way but I have created 3 text fields (applied PHP action) to each of the along with a submit button (applied Action.) I am able to get it to send an email but their is no contact. I am guessing that these all need to be inter-connected but not sure how?

I.E. I have text field Name and Email both with the action applied, and I get a blank email basically from the server. Is there a form I am supposed to set up first? Am I supposed to create a HTML box and do within. Please help, I am lost on this and would appreciate any help.

Hi funrivers,
Try this;
Add your three form fields to your page as you would normally in
Freeway ensuring that each is not set as a layer item. I tend to
insert these into a table structure to hold things together but then
I’m possibly a bit old school when it comes to things like that! Apply
the PHP Feedback Form action to a single parent object - either the
parent table (in my example) or page. Use the actions palette to set
the success and error pages for the form and specify the e-mail
address the form should get submitted to once completed.
That’s it! If you publish and upload your site to a PHP enabled server
your form should now work. Take note of the special names for the text
fields. If you name these ‘name’ and ‘email’ (without the quotes) then
the PHP code generated by the action will validate that the e-mail
address is correctly formed and require the name field is not empty.

taken care of by the form. I’ve been using the form as instructed, but
am so far having lots of problems.

One of these is if you put two email addresses in the ‘to’ field it
only seems to send to the last address. Another is that my form
contains two fields for name: First Name and Surname. If I put ‘name’
in both fields will this confuse it? At the moment I’m only putting
‘name’ in the Surname field.

I must be doing something wrong.

I am creating 3 text fields… inputting name and value… then a button (submit)

Then making the page PHP Feedback Form… but doesn’t appear to work.

Now if I add PHP Feedback Form to each Text Field and Button … then I do get an email but blank form… So the server appears to be PHP enabled… using Blue Host…

I like the idea about a table but how would you insert a text field or make it as an input function on the final result?

Nevermind,

I figured out… I wasn’t clearing out the Value when inputting Name/Email on the published/uploaded site…

In other words… Name: i thought you wrote name or email after the Name:/Email: part… not replace it… Something really simple yet took me a while to figure out.

Hi Dick,
The page your form is on can have any extension you like (.php, .html, .shtml,
etc) as all of the form processing is handled in a separate PHP file that the
action generates (*-go.php), uploads and submits your form data to.
The two e-mail addresses in the recipient field error doesn’t surprise me but
it’s a good suggestion and I’ll make a note of revising the code for the next
version of the action. The name field is used to construct the sender’s details
in the incoming e-mail and, again, this could be split as you suggest. I’ll have
a think about how I can do this in the action code without breaking things.

I keep getting server errror on php feedback form. and I also wonder how to get fields mandatory.
Here is my test page: advis - hemsida och logotype, ditt företags viktigaste kommunikationsverktyg !.

Hi Ulf,
The PHP is returning an error either because it is having problems sending the
mail itself (some ISPs will restrict access to the mail function) or because
the version of PHP on the server doesn’t like some of the legacy PHP code the
action creates.
What version of PHP is the server running?
As for making certain fields mandatory, try using the form validation suite
(http://www.actionsforge.com/projects/view/73-validate-form-suite).

Will the Form Validation Suite work with Freeway Express?

Also, how can I get the form email that comes to me to have spaces between the items? It comes in line-by-line, all one after the other. I’d like some white space between some items when looking at the contents in e-mail.

Hi Helen,

The Validate Form suite is a Pro only Action, to use it you would need
to upgrade. We offer a substantial discount for existing users at our
store.

To edit the way the text comes in from the form you will need to edit
the PHP in the Action. You would also need to use Pro to change
Actions and install them yourself.
