Check to make sure you only have the one version installed. Edit / Actions from the main menu, look at the list. If this Action is being accessed from the document’s cache, that could be a source of this problem. It’s highly unlikely that there is a bug of that kind in the Action itself. You might want to try restarting your computer, and possibly close all documents and open a new blank document and see if the problem travels there.

Hard to say. The file is provided as a Zip if you downloaded it from ActionsForge, so it may be the fault of whatever application Chrome chose to use when un-zipping it.
