Have we lost the ‘Site Folder’ structure in FWP7?
Also, are index.html and home.html interchangeable in the sense that just specifying a folder will load home in the same way that index does? For all browsers? Or is it best to rename home to index?

are index.html and home.html interchangeable
They are on some server setups but this really is a question that your hosting provider has to answer.
The 'nix norm is index.html, index.php etc but can be configured differently.

Nope - what are you experiencing, what have you set as your ‘site’ folder under the Document Tab in File>Document Setup

Sorry, I was being a bit stupid there. If a site is created from a template, then it is saved/set up to ‘site folder’, but creating from scratch requires the ‘site folder’ to be set up manually.

That has always been the case as far back as I can remember.
Working from a Template creates the Media Folder and the Site Folder for you.
