[Pro] E-mail form

I want to insert a form on a web page, "Contact Us’ to get feedback from customers. Is there an ‘action’ for this? The form would have 4 boxes to contain Customer Name, Their E-mail address, Telephone No. Message Box and a Submit button.

Submit button could be a roll over with a simple hyperlink to my domain e-mail I guess.

Any guidance would be appreciated, please keep it simple.
