You need to upload a copy of the license to the same folder the form
is in, in the case of the link you sent through the license should be
in the home directory of your web site.

I see you are not using the easiForm actions, at least you haven’t
used the PHP easiForm Elements action at all, the form will not work
if you do not set it up correctly.

It will take you 15 to 20 minutes to go through the simple tutorial
and set your form up as outlined there, it will not work at all if you
do not set the form up as directed.
BTW, Nothing to do with the license, just a couple of comments on the
form if tha is OK:
If you select the item the easiCaptcha is applied to you can then
set a style to it so the text output is the same style as the rest of
the form.
You have ‘contact phone’ set as a required file ‘at least with the
Javascript actions you are using’ but it is not marked as such (no
asterix).
You don’t seem to be using the PHP easiForm Errors action, you do
not need to use it but it would give some friendly feedback if the
match captcha was not correctly selected.
Might help the form layout if you closed the spacing up between the
form elements, looks very spread out at the moment. It would help to
set it up in a table. The extra time of building your form in a table
layout may save you a lot of time in the future trying to get it to be
laid out on the page neatly.
