on the top left of my form. Anyone know why and how to resolve this?

Hi Jay,
It looks like this code has been entered manually. Is there anything entered under Page > HTML Markup or in Page > Form Setup > Hidden Fields?
The PHP Feedback Form Action holds this sort of data within the -go.php that it creates so from a user perspective you can never see the recipient’s email address.
