Yes, but only in Webkit browsers. You could make the form exactly the
same, except not rotated at a jaunty angle, in any browser, without a
lot of effort.

Add your form elements as inline items within the run of text. So it
would read “we are getting married on [form element date] at [form
element time]…” The rest is CSS. Remove the borders from the form
elements, leaving only the bottom border. Make sure that the font is
the same inside the form input fields as outside, etc. Tweak and test
in multiple browsers. Submit button (post it…) would be an image
submit button.
