What causes pages to publish whether they have been changed or not? Repeated/consecutive upload commands cause all pages to be published each time. Is this a possible cause of a responsive/inline design?

I doubt that it has anything to do with the design. When you say consecutive uploads, do you mean that you upload the site, then immediately upload it again?
Things that cause a full upload include global stylesheets with changes (since they link to every page, and cause the parent page to become “dirty”), changes made to master pages, changes to your source files (images or other linked content), and finally, clock drift. If a Mac is not set to synch to the global NTP standard, its clock will eventually drift away from the true time. Servers almost always are synched to NTP, and so they may have a different idea of what time it is than your Mac. (This is not standard, and not usual, but it happens enough that I mention it. Macs are set to use time.apple.com out of the box, so you have to tell it to do otherwise.)
