I would really appreciate any help with this Feedback form. It might be something simple but after 6 hours of trying I am out of any new ideas.
Can’t make it work. I noticed that a lot of people had problems with it. I am one of them.

Getting this message from the server:Not Found
The requested URL /gdform.php was not found on this server.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.
Has this feedback form ever worked on this server before?
If the answer to question 1 is yes then you need to contact GoDaddy and ask them to reinstall the gdform.php file that should be present at the root of your account. According to the GoDaddy support documentation this should be present on all Linux packages but (from my experience) is often missing.
If the answer to question 1 is no then turn off the GoDaddy option in the Action, upload and try the form again.
If the answer to question 2 is yes then your host has changed something on the server. Again contact them and ask them what has changed to make PHP form to mail fail like this.
If the answer to question 2 is no then I’m hoping that the answers to question 1 will resolve the issue.
In a not too distant future there will be a shiny new version of this Action with far fewer features and a core support for most Apache web hosts. Sadly GoDaddy won’t be on that supported host list as unless they do something to dramatically improve their form to mail processing in the near future. I would estimate that 90% of the support emails I answer about this Action involve GoDaddy.
My (as yet unpublished) FAQ on the Action starts with;
Q. Are you planning on hosting your site on a GoDaddy server
A. Don’t!
Once you’ve the script installed and setup use the GoDaddy checkbox option in the PHP Feedback Form Action interface. It will set your form up to post to the gdform.php script.

Yes. By replacing your existing form tag with the code below, you can upgrade to webformmailer.php without any additional changes.
