Hi Ian,
Looking at the code for that page I can see that you have what look like two forms (a feedback and a tell a friend) with two submit buttons but only a single instance of the PHP Feedback Form Action applied. Firstly make sure that all your layered items are turned into non-layers. Also make sure that your items have unique names (take a look at the product page in regard to special names - http://www.freewayactions.com/product.php?id=019).
Now apply a copy of the action to an item that encloses the first form (the feedback form) and set the parameters in the actions palette. Repeat with the tell a friend form and you should be set.

Hi Ian, If you still have the problem after using Tim’s fix, I encountered a similar problem which was resolved using a soft stop (shift-enter) after typing in the html box “Name” and “Email Address”. Also, you may want to take a look at the Freeway Moments podcast - Easy Feedback Forms.
