[Pro] PHP form help or suggestions

My customer would like me to create a form like the link.
I have used a few different form builders in the past but am wondering what I should use if I want to remain responsive or is that asking too much for Freeway. Can I use a table and the Send Form Action?
Or is there a better way?

This form would be most easily laid out using a table, for sure. But SendForm does not care one bit what your layout looks like. Its only concern is writing the PHP (server-side code) form handler. By default, it also uses the HTML5 form error tools built into modern browsers, so the error messages will actually be native (drawn by the browser itself) and again should not care about the layout technique used.
