[Pro] Please help with contact form

Hi on the home/index page of my new website i have put a quick contact form so i can call clients back,
but for some reason it comes up with this page with loads of code on when i press submit, i have checked everything and i just can’t understand it,I have made numerous websites and had no problem with contact forms before if anyone can please help i would be very thank full

Not seeing that here

BTW you are not using the Placeholder attributes in your Form Fields correctly - you are adding values into the fields which do not disappear when you click inside.
