Where it says “schedule a consulation” and " have us call you" I want that to be a pop up with a form so people can fill it out. How can i make this happen

You make your form on a page in your document. The form should be the only thing on that page. Then, in your other page, you create a link to that page, and apply the SL Action to the HTML box containing that link.

Create your form on a new page in FW - size the page to fit the form.
Now apply the action to an Html item (or graphic) that contains a link to your form page.
