Just tried mine and when adding the send form action to the page the dialogue appears in the actions palette. Have you opened your actions palette so it is showing? The dialogue will then appear in it. I know this works perfectly well in 7.1.4 as I have had a voting form made with this action for a month and it worked perfectly.
