What method are you using to upload the images? There are two possible reasons why a file isn’t loading – either it is physically missing, or the file that is there has the wrong permissions – not world-readable – and so you aren’t allowed to see it.
Using Freeway’s automatic upload is one of the best ways to handle both of these problems. If there are no other files in your site root (placed there manually by you or your ISP, unrelated to your Web site itself) then the simplest thing to do is use a third-party FTP application to delete everything from the inside of your site folder on the server (variously named htdocs, public_html, public, etc.) and then allow Freeway to upload the entire site again. This can clear out issues that are otherwise difficult or tedious to diagnose, and unless your site is huge or contains a number of high-res movies or similar, won’t take more than a few minutes to complete.

The images are uploaded to my provider and, so far as I can tell, properly placed in FW.
