There are a couple of Actions out there that you can use, I have PHP
easiForm which has an add on action so you can use it with PHP
easiCaptcha. easiForm and/or easiCaptcha require a small fee for a
domain license (bundle discount) and your server needs to be ioncube
friendly.

You can experiment setting up your form and even get emails in the
demo mode of easiForm but an alert will show rather than the thanks
page for the easiForm action, so you can try before you buy.

if using easiForm as most user problems come from not reading how to
use the actions):
