Send form weirdness

I have a bunch of pages on my site using SendForm action. They all display and work perfectly. I needed to duplicate one of the pages, and change one detail. Every time I do that it goes completely weird. I have tried to simply duplicate the page several times, but same result. Strange. Cant figure out what is going on:

Try deleting everything from your Site Folder (on your Mac) and publishing again. What you’re seeing is Freeway getting confused about the path to individual image resources. Deleting everything that Freeway published into the Site Folder on your Mac will force it to re-create all the resources from scratch, which should clear this up for you. There’s also a hidden menu option, if you hold down the Option key and choose from the File menu, the Publish command will change to Publish Everything. If you feel nervous about deleting the contents of your Site Folder (and you should not) then that’s a half-measure you can take first to see if it makes a difference.

Thanks. I did as suggested - emptied the Site Folder, republished. But, although I have republished a bunch of times, the Site Folder now remains empty…??

If your site folder remains empty then you must be publishing your site to another site folder elsewhere. Check in the document setup that you have the correct site folder selected. I tend to include the name of the website in the site folder name which makes it easier to identify.

Ok. Back in Australia, so I completely republished my site folder, cleaned out the folder on my server, and put all the items from my site folder into it. Same problem. I even deleted the page in Freeway, and redid it several times. All my other intake forms are perfect. I cant understand why this particular one is so weird:

Argh. I cant work it out. I have completely clearly out my Site folder, re-published. Cleared out my folder on my server, put everything again onto there. Still problematic:
