Background images don’t get alt tags at all, because they aren’t
actually img tags. The img tag is the only tag that gets an alt, IIRC.
Background images are CSS properties only, so they’re not actually
“in” the page in any sense of the word.
Images placed as pass-through are normal “foreground” images, and thus
do get alt tags (and you can see/update these in the Inspector while
the image is selected in your layout).
