I get what you are saying… I was thinking the same…
I was thinking - they purchase and then they get an email with the form or a link to the form to fill out… I have to pass this info on… I think what they are worried about is not getting the info…

My feeling on this is that there is too much information gathering going on and too much time for the donor to have second thoughts.
I appreciate that a lot of this info may be necessary but you don’t want them to be put off.
That is why it makes more sense to collect this info after the commitment has been made.
Not sure of the best approach…get the money first and then get the info?
Could the info-gathering and donating be separated?
Register people at the website - newsletter, updates, whatever. Gather what information you can there, then get the rest through the donate form? Just an idea.

Make only two choices on the page. One for Standard Memorial and one for Veteran Memorial. Both presented with appropriate beautiful graphic support. After choosing you get the proper form in a popup, slider, page whatever.
The forms can appear much less intimidating if designed in separate blocks of information with supporting graphic touches that inspire relevant emotion. This will inspire the user to press on.

My choice would be two buttons on the main page for simplicity. I do popups like this using Spawn New Window with iPad friendly checked. You can put a form on a page and load that in the Spawn New Window target. I’m a real fan of jotform.com because of features and ease of use and you can update and change without ever having to fire up Freeway. The site owners could even do that themselves. Jotform has payment gateways also. But, you can load any page in Spawn New Window with whatever form you create.
