This is driving me mad…admittedly FW3.5, but doubt things have changed that much.
Because my site is large, to speed up working and saving i’ve split it into two sites. Because I’ve split it into two sites I have to be careful to name graphic items uniquely or FW will just apply an ‘item16’ name to it…but that means there could be two quite different item16’s and you couldn’t have that in the final Resources folder.
So I’m changing some graphic item names to meaningful names, but FW just won’t publish the new name… I change ‘item6’ to ‘greybox’, save and publish…but FW still publishes item6.gif
It just seems to be ignoring the title of the graphic!
Anyone had this problem before or know how to fix it?
Simple, obvious, but has been missed by others - including me. After changing the file name, did remember to tap the Enter key?
And by the way, there is a great deal of difference between FW 3.5 and FW 5.5. Good stuff that makes life easier and well worth the price, though you have to watch a couple of things (mainly styles) if you open an old site in version 4.0 and above.

Yes, tapped the enter key!..I’ve learnt that one, too.
Hear what you’re saying, about new versions, but it SHOULD work in old versions - the functionality was there long before 4 and 5 were even a twinkle in the ye of the Softpress developers
