Incorrect hreflang implementation

Although I have a very good ranking, google sent me this warning:

Incorrect hreflang implementation

How can I repair it? As a string in Head?
Are there any other solutions? Thnaks for Help.

I just found it out. I’ve tested it with systrix (dot) de. So actually I could solve it. (Used a hreflang online tester)

BUT I have another question: the 2nd error was, that our site is found under “name.ch” and “name.ch/index.php”. But there is only 1 on the net…
