That depends on a lot of factors - what are the criteria?
This example presents a pop-up when the page is first visited and uses a Cookie to ensure you only see it once - built entirely in FW.
Your needs may be totally different.

Hi, sorry for the late input. I handle these things differently, by only serving the form when the user requests it. I use CSS to hide the form and leave the user a link to request it. There are a couple ways that I do this… present the form as part of the body content, or present it as an overlay.
