[Pro] Blank form returned

I made a simple form in FW Pro 5 following Creating a Contact Form in Freeway - Freeway - Softpress Talk

There are just four simple rows (Name, email, message, then the submit button) and two columns. Column 2 is are the text fields with names.

Uploaded the file and tested it. Received the following email:
The following information was submitted from a form on (my email address)

submit: Submit Query

I undoubtedly have left out some simple step somewhere but cannot find out what that might be. Suggestions?

Hi Bill,
It looks like you’ve applied the action to the submit button. Apply it
to a parent element (either the Item1 div, the table, or even the
page) and you should be set.

So far, no luck with applying the action to the page or table. Now I am getting an error message.I will keep playing with it.

In the meantime, is there a way to remove an action from an item or page? I have tried deactivating it, but it still shows up on the page.

I took it off the submit button by deleting the button. Now I have it (PHP feedback form) on the top of the page. No big problem for me to delete the page, but is that really necessary?

If you have not figured it out by now…I am new at working with websites.

Hi Bill,
Show the actions palette (Window/ Actions) and click on the little
grey x in the action name tab at the top of the palette. This will
remove the action instance from the item or page (which ever was
selected).

did that. but the actions icon still shows. maybe I am assuming too much. I would have expected the actions icon to have disappeared from the name of the item.
