I am able to enter all information correctly, but form keeps going to error page? Cannot see what is wrong. I know it’s some simple little thing I’ve overlooked… again.

I’m getting the server failure error page. Looks like your host has specific requirements that haven’t been met. This is often to do with the email address that you’re sending to – a lot of them now require it to be a address that they’ve provided. If this is the case then you’ll need to create an email address with them (if you haven’t already), send to that, and set up a forwarding rule to the address you want to send it to.
There are other requirements but each host is different so you’ll need to contact them to find out what they are. For example, if it’s GoDaddy, they don’t allow you to use any form handler scripts other than their own. There’s an option to use theirs in the PHP Feedback Form Action.
