Internet basics … the default for all domains is index (.php/.htm/.html … etc).

Almost always true, but the real answer is “whatever your sysadmin has set it to”. If you have to deal with a Microsoft IIS “server”, then you will change it to default.htm (no l, like it’s always Christmas).
