When I choose CMD-D for my graphic item, FRW only duplicates it O N C E, instead of showing the window: “Horizontal offset …, the Vertical offset … and the Number of copies …”.
This I don’t get, so I cannot stack graphic item on top of each other.
What version of Freeway, what version of Mac OS X? I don’t see that
behavior here. Have you done anything in the Keyboard control pane
(System Preferences) to set a shortcut to Command - D?

Command-D opens the duplicate dialog unless you have an inflow HTML or inline graphic item selected, in which case it will create a duplicate below or next to the original (depending on its float or alignment settings.
