[Pro] Fix PHP Feedback, create another problem!

Thank you Walt.
Regarding the PHP Feedback Form…
I changed the tag from php back to html as you suggest and it worked.
HOWEVER… This time, upon first opening my file, even before I could fix my original problem, I found another problem.
My CSS Menus is not functioning properly.
When I go to preview, I get a message that says…
Actions has JavaScript ERROR.
The following actions will be ignored as they have a known JavaScript Error.
Action {CSS Menus”, page “Contact Us”
(This message appears no matter what page I go to. The name of the page in the Quotes changes.)
Do you think this is a coincidence? Or, is it related to the mistake I made in creating my Contact Page?

You might want to remove all Actions from the page, publish once, and
then add them back again. Also, do you have any of these Actions
applied at the Folder level, or through a Master Page? Might be useful
information.
