Knowledge Base reads: Open the Page Actions menu from the Page menu and select PHP Feedback Form
5pro don’t have that option. Lost.

I’m trying to activate the form from the mobile template first and then modify it to my needs, but I can not get the action palette give me the options.is blank.

If I go to actions, opens the action palette window but it does not have the settings to be set.

Copy and paste the form table into your FW doc.
Remove/delete the fields you dont need.
Keep the Send button
With the Table selected you will see the settings in the action palette.
Create an error and a success page in your FW document and select them in the action interface.
Put the email address you want to receive the info in the Recipient box.
And that is all you will need.
Upload and test.
