I am not sure if I can trust Xcode as the simulator of reality, but obviously it loads the dedicated image:
The “smallest” as the fallback (fallback) is as well way better (however it looks a bit weird in the FW-doc 
Regarding Landscape/Portrait I’m not really concerned, cause my breakpoint philosophy is anyway slightly different:
1200px for Tablet - 600px for SmartPhones which will mean:
The same situation on both - or:
I’m not after devices, I’m after device-ranges.
It is still a confusing concept (to me) at all and I have to review this (your example using php to include as well). And I’m still not sure how funny life will be creating 6 versions of a single image (3x for breakpoints and each a 2x version for Retina) - especially in slideshows and galleries.
