Use Radio buttons and not Checkboxes, you should have the Radios as a cluster, basically several Radio buttons relating to one group should be names the same then the ‘Values’ for that group ‘Cluster’ would then have a different value.
If you wanted to do something similar elsewhere on your form then you would just name that group ‘cluster’ differently and the two will not conflict.

You should use a radio button group instead of a checkbox group if you want an exclusive choice. Checkboxes are for multiple (yes, and…) choice questions.

Basically, there’s a Question on the left, and choices next to them on the right (none, good, very good, etc. I have each of those choices in a separate cell.
When you say, cluster, you mean all in the same cell?
No, all having the same name ‘Name:’, this then creates a group ‘or a cluster’, each of these Radios would then have a different value ‘Value:’
