I have successfully use Tim Plumb’s php Feedback Form in the past, but now it is giving me a major headache.
At first I could not get it to show the success page. So, I applied it to the table instead of the page. Now I get the success page, but nothing is sent to my email.
Any help figuring out why this is not working would be greatly appreciated.

The form itself appears OK but you should name your fields correctly.
Under the 3rd Tab these should be (case sensitive) ‘name’ and ‘email’
Where is you site hosted as this may be a sendmail issue
Does your hosting provider give any tips about form processing?

George, just a quick observation.
If this is your intended design, it looks like it would be a perfect candidate for Caleb’s responsive template called Backdraft. Why not give it a shot?

I think he means that the mail responses were in the Junk mailbox ie flagged as Spam.
