This worked a few times, but never went to a success page only to a 404 page (even though I have success and failed pages set up within the PHP feedback form options) now its not working at all - I have ticked the use Godadday mail script and followed the set up the Form Mail with Godaddy to send to a specific email address.
Any ideas?

The form wants to post the data to GoDaddy’s mail script which should be here;
http://www.ayearofkindness.com/gdform.php
…but it isn’t. Open a support ticket and have GD add the script back to your account.

Its weird I can see it but when I tried to open it it said that the file is not there I’ll get go daddy on the case
