Unless you changed something in the form, the first place to look is at your hosting provider. Are they GoDaddy? Did they recently upgrade php?

Hmm… That’s odd, this sounds similar to the post I just made! http://freewaytalk.net/thread/view/136654
We had a form using PHP Feedback Form Action that’s been working for over a year now and today it stopped.
Obviously I understand there’s little if anything linking our sites, unless the last PHP update broke something?
