Probably because you have jQuery on there as well.
It so happens you are loading 2 versions of jQuery - never a good thing never mind the javascript library conflicts when using Script/Protaculous on the same page.

So I removed the scriptaculous stuff and the old version of query but it still doesn’t work.
Also every time I upload to the server Freeway is writing this form file over and over -FW_SendForm_index_contact39.php. I’m up to 39.

Go into the Site folder and delete all versions of -FW_SendForm_index_contact?? - at the next upload FW will start again and only produce 1 and delete all the other versions from the server.
And yes 1.11 is the newer version.
Your error probably comes from the load order of your (many) scripts.
Try adding
Before the jQuery no conflict but you may have to play around with this - not sure which rely on jQuery!

I removed all scripts one by one to see who may be the culprit but that did not fix the issue.
Lightbox was completely removed from the server.
Very odd.
