My sent form has been built many times with the difference settings an uploaded. I always get the error page on the site. PHP (latest) is available on host.

Try to use the PHP feedback action on page - it does not work!!!
One advantage of that action is that you can show a different error page depending on the type of error ie you can set an error page if there is a server failure (processing the form) - this greatly helps with diagnosis.
So have you tried a very simple form (nothing else on the page) with just an email entry field and a submit button and set different error pages for each error. That would tell us a lot.
